A comparative study between clinical signs and electrodiagnostic examinations in carpal tunnel syndrome


Abstract
The objectives of this study were to evaluate the diagnostic sensitivity and specificity of each clinical signs compared with electrodoagnostic examinations, and to evaluate the severity of pathology with needle EMG in carpal tunnel syndrome.
The subjects of this study were 34 cases(47 hands) with clinically suspected carpal tunnel syndrome who underwent electrodiagnostic examination from Jan 1,1988 to Sep 30, 1992.
@ES The results were as follows :
@EN 1) The most common clinical sign was hypesthesia(76.5%) in clinical diagnosis of carpal tunnel syndrome.
2) Electrodiagnostic results were 24 cases(70.6%) of carpal tunnel syndrome, 8 cases(23.5%) of cervical radiculopathy, 2 cases(5.9%) of normal in clinically suspected carpal tunnel syndrome.
3) The distal motor latency of median nerve was the most prolonged in Phalen sign positive group and the amplitude of median MUAP was the least in muscle atrophy group.
4) The needle EMG on APB muscle showed the highest abnormalities(83.3%) in Phalen sign positive group and the most severe involvement in muscle atrophy group.
5) Phalen sign positiev group showed the highest diagnostic sensitivity and specificity.
With the above results, the most sensitive and specific clinical sign in carpal tunnel syndrome may be considered Phalen sign so that Phalen test should be included in clinically suspected carpal tunnel syndrome.
